Notice to Vacate

1. Submit this form 15 days prior to the expiration of your Rental Agreement (anticipated date to vacate).
2. Have paid all rent (and any additional charges incurred) THROUGH THE LAST DAY OF THE MONTH IN WHICH YOU GIVE US NOTICE.
3. Have met all other terms and conditions of your agreement.
4. Leave your unit SWEPT CLEAN, UNDAMAGED AND COMPLETELY EMPTY.
5. Haul away trash. RENT DOES NOT INCLUDE TRASH SERVICE.
6. Vacate before 12:00pm of the last day of the month during which you submitted this notice.

***NOTE***
- Operator may re-rent this unit immediately after move-out.
- Occupant fully understands that any goods left in the unit after date given below as vacate date will be considered abandoned and will be disposed of, with a cleaning fee charged to occupant.
- Operator does not refund unused rent. Move out payment is through the 15th or last day of the month. We pro-rate when you move in - not when you move out. You have signed a monthly lease.
